Philanthropic Contribution - $1000.00 Supporter of The Goals of the Association for Integrative Healthcare Practitioners. (Advocate for Natural Health Care and Education - Freedon in Health Care Choices) ($1000.00 + $0.00 = $1000.00 2. Collegiate Membership - $800.00 Educational or Training Facility Credentialing and Certification. ($800.00 + $75.00 = $875.00) 3. Medical- Clinical Facility Membership - $800.00 Medical or Clinical Facility Credentialing and Certification. ($800.00 + $75.00 = $875.00) 4. Professional Medical Practitioner Membership - $250.00 Licensed Medical Professionals Credentialing and Certification.(Ie:Physicians,DDS,DO,Chiropractors,PA,NP,RN) ($250.00 + $75.00 = $325.00) 5. Allied Health Care Practitioner Membership - $200.00 Allied Health Care Professionals Credentialing and Certification.(All other Health Care Disciplines) ($200.00 + $75.00 = $275.00) 6. Herbalist Membership - $400.00 Herbalist and or Herbal Pharmacy Credentialing and Certification. ($400.00 + $75.00 = $475.00) 7. Retired Practitioner or Provider Membership - $100.00 Retired From Health Care Occupations ($100.00 + $75.00 = $175.00) 8. Student Membership - $110.00 Student of Integrative Health Science (Must be attending (Full Time) an approved Program) ($110.00 + $75.00 = $185.00) 9. Administrator Membership – $250.00 Executive – Administrator for a Integrative Health Care Clinical Facility or Program. ($250.00 + $75.00 = $325.00) 10. Educator Membership - $250.00 Professional Educator of Integrative Health Sciences. ($250.00 + $75.00 = $325.00) 11. Distributor Membership - $600.00 Natural Health Product Distributor or Vender.
